PARENTAL OR LEGAL GUARDIAN CONSENT: PLEASE READ AND SIGN THE FOLLOWING ACKNOWLEGEMENT I give my consent for my child to participate in Generation W, Inc. (“Generation W”) programming, including, but not limited to, Generation WOW group events, WOW School Clubs, quarterly excursions, and community service projects. I understand that my child will be assigned a mentor who will communicate with her on a regular basis, potentially via phone and/or email. I fully understand that the program involves mentors. Caring and dedicated women representing diverse community, business, and entrepreneurial backgrounds will serve as mentors for a minimum of one year. Further, I understand that the individuals who serve as mentors in this program are volunteers and are not employed by Generation W. I understand that my child will participate in an orientation session at the annual Generation WOW group event in which the program will be explained. I understand that during the course of the mentoring program there may be special group events (incorporating all mentors and girls). These events are supervised functions of Generation W. I will be specifically advised by Generation W as to whether an event is a supervised group function. I give my consent for my child’s school records to be accessed by Generation W through my County Schools’ Student Information System for the purpose of gathering data for analysis of program effectiveness. The data collected will not be used to identify any individual child. I irrevocably grant permission to Generation W to photograph, take motion pictures of, or televise, and reproduce in any manner or through any media (including but not limited to social media), my child’s image and voice; and to use, display, license my child’s first name, likeness, voice, appearance or image in motion pictures, telecasts (all referred to as “Intellectual Property”) without compensation and specifically acknowledge that Generation W holds all intellectual property rights (including but not limited to sole copyright ownership) in such Intellectual Property. I understand that by signing this form, I am agreeing to let my child participate in Generation W programming. I understand that even though Generation W will provide reasonable care and supervision during all activities, there may be risks inherent to participation in our programming which cannot be avoided or eliminated. I further understand and agree that activities outside of the specified group activities (for example, any individual or small-group meetings between my child and her mentor) shall be solely at my guidance, approval and option and Generation W, Inc. has no ability to supervise or approve such activities. By signing this form, I waive the right as parent/guardian and on my child’s behalf to recover from Generation W, Inc. in a lawsuit for any personal injury to my child or any property damage that results from the risks that are a natural part of this programming or that occur in all activities outside of Generation W’s supervised group activities. If I become aware of any non-supervised group activity concerns, I agree to immediately advise Generation W of such concerns at (904) 372-4859or info@genwnow.com and to remove my child from such activities until my concerns have been addressed. I understand that I have the right to refuse to sign this form, and that Generation W reserves the right to refuse my child’s participation in its programming in the absence of my consent to these terms and restrictions.
